The Department has produced a series of information sheets for doctors,nurses, those delivering personal health budgets, allied health professionals, health trainers and anyone supporting individuals with long term conditions. The information sheets cover a range of topics including care planning, care co-ordination, managing need and assessment of risk, motivating people to self care, goal setting and action planning and end of life care.

The Department of Health has published a guide on emerging evidence, including examples, to illustrate how the principles of personalisation have been applied, emphasising the value of finding ways forward that make sense and work best locally.

Putting People First puts the needs and views of service users at the centre of any service, provision or planning process. It also requires service providers to work in an integrated way. This Skills for Care 'Outside the Boxes' toolkit was produced in response to a request from providers in Yorkshire and the Humber from various care settings for support in developing integrated services.
The document gives examples of where integration has been a success both regionally and nationally and provides some 'how to get started' tools for all sizes of organisation in all care settings.

SCIE have worked with the Voluntary Organisations Disability Group (VODG) to develop a tool to help you assess how ready you are to deliver personalised care services. It will be of particular use to social care providers.
This is the first stage of the development. The second stage, which will be launched in December 2010, will direct you to resources that will help you to make improvements.
The tool is straightforward to use. It will take you through a series of questions under four section headings: Person, Team, Organisation and Market.

Skills for Care North West
In April and May commissioners and providers were brought together for two events looking at personalisation in the sector. The event featured group work and allowed commissioners and providers to identify the likely impact, key actions needed and recommendations.
